In a medical report, I'm confused by the "por".

We attempted to remove the urinary catheter without effect "por" pouch bladder.

Can anyone make it understandable?

Antagonic actions A "globo vesical" is an overdistended bladder due to urinary retention, which has many causes. This situation can be solved by the insertion of a catheter to drain the urine. If said catheter is left in place for a time, it can become lodged and its removal can be tasking, especially if it has an inflatable cuff that apposses the bladder neck.
It is not the "globo vesical" that renders the catheter irretrievable. Moreover, there shouldn't be any vesical distention if the catheter is permeable, because the intention of puting it there is precisely that: prevent the retention of urine.
I think the original writer has concepts that are antagonic.

I've just found in google:
En el supuesto de no poder retirar la sonda vesical, por imposibilidad de vaciamiento del globo vesical....

which seems to be a more complete version of what my report is saying. Can anyone confirm that this is what they really mean, or am I looking in the wring direction?

well, the client provided me with a glossary of medical terms, and that is the translation they give for 'globo vesical'. If its wrong, then I still need to know how the sentence should read in English
I am also confused... But not by the "por". "Globo vesical" is a suprapubic mass felt when the bladder is massively distended due to urinary retention—where does "pouch" come into this?
