: This is what it is (We have one for my daughter), but perhaps it may be better to add "official" or "notarized" to highlight that this is not just a note by one parent signed on the back of a cigarette packet.

Good point! Though in most cases such documents are to be certified as a true copy by a solicitor anyway.
