Er ikke helt sikker på hvordan jeg skal oversette "HJELPETAKST" med.
Det er mulig at "SERVICET" er forkortet fra "SERVICETAKST" (Service Estimate?).

The sentence on the form is "Skadetype: HJELPETAKST/SERVICET"
The form is an estimate form done by the Auto shop related to insurance. Other fields next to this field are: "Skadenr.", "Skadeomr.", "Skadedato".

Can you post the sentence? It might translate into Service Bracket, as in which level of service you are paying for, but there are two other terms that come to mind too.

repair estimate

As you may know, "hjelpe-" is sometimes used as a prefix for remedial measures, e.g., in "hjelpemidler". What you have here is a form that provides for listing either a repair estimate or a service estimate, depending on the appraisal to be done. See the link for a use of "hjelpetakst" for the example sentence.

Service Plan

Normally used when talking about warranties, it is a service that extends warranties for a fee.

I will admit, I am not on firm ground with this translation. I cannot reference you to any texts, so I think Charles Ek has the better translation.
