There are many forms of deformity, the most common being Twisting Deformity. This form occurs frequently with Dyslipidemia.
I translated 表われやすい as "occurs frequently" rather than "occurs easily" which is the literal translation, but does not convey the meaning in English.
高脂血症 was changed to 脂質異常症 in 2007 (see Wikipedia link below).

For your other terms, I would put:
Looped form with dense end/tip
form with bypass on the side
Looped form with thick lower area
I use http://babelfish.yahoo.com/ for individual words sometimes, but you can't expect a machine to ever take the place of a good translator! 変形 is deformity/deformation, NEVER "transformation". When in doubt, check multiple dictionaries. Online translating software will only give you one option, and that may not be the most common or best suited for your context.
