This is part of a set of instructions for a questionnaire. I'm just not clear on 則して here. Are we asking them to show the data "in accordance with Form D", like a corruption of に即して? Thanks in advance.

The way I guess is that Form D is shown first, and there are instructions in the Form as to how the data be presented.
即してis "てform" of verb 即す, which means "based on".
Yes you can say "show the data in accordance with", or "show the data accordingly".

Woops, it is 則して not 即して. OK a little adjustment. First thing first -- the former is not corruption of the latter. However they are pretty much the same in a sense ¥"something must be followed or obeyed¥". A bit more Japanese lesson, though. This Kanji 則 sometimes used as 則って(read as ¥"nottotte¥"). Nonetheless, this is a bit archaeic. This means ¥"observe¥", ¥"act upon¥", in other word ¥"obey¥". So you would not take a liberty of any form as you wish, but the Form D.
So here is my conclusion.

Thanks. In my experience, 即して is more commonly used by my clients. Actually my WinXP doesn't show 則って as a henkan option.
