ATTORNEY AT LAW is a person who received the degree and is authorized to do all that a "real" lawyer can and is allowed to do.
ATTORNEY IN FACT is a person authorized representative someone else through Power of Attorney, the authority given to him/her. It can be a Lawyer - or anyone else.
In the US, a notary public does not have to be a degreed lawyer. Anyone: bank clerks, secretaries in mortgage banks or private citizen can be Notaries - AKA Attorney in fact.
