Hi, everyone! I'm trying to make sense of this phrase and I've come up with two different meanings. The text belongs to a marriage certificate, and "he" would be the person authorized by law to perform the marriage.
"he is required to return his certificate in the form hereto annexed."
Would it be: tiene la obligación de devolver/devolverá este certificado conforme al anexo adjunto.
or.. devolverá este certificado utilizando el formulario adjunto.
I'm not sure if "form" refers to "the correct way" or "a form", as in "formulario".

This leaves it a bit in the middle. I do think it's a standard form (formulario), but possibly you may also use the form as a reference.

Habla de algo anexado, o sea de algo físico, si fuera una manera de hacer las cosas diría algo como "as in the annexed model".

Me refería algo anexado que debe completarse con los datos de la pareja, y que dice "IN the form annexed" y no "AS IN the form annexed", o "as it's shown in the form annexed".
