Could it then be a last name (familienaam): van Voor? Admittedly not a very obvious or good choice for an example but that name does exist.

Names like Van Houten, Van der Bilt, Van Heukelum, Van de Molen etc. should be written like this: Houten van, Bilt van der, Heukelum van, Molen van de etc.

It looks to me like 'previous' with lots of emphasis (all caps and letterspaced -- all relics of the typewriter era).

Just a guess.
Maybe the person filling in the form, is asked to tick the box to specify if he is the actual account holder or rather a person acting on behalf of the account holder.
